transfer back to micro SIM?

I need to transfer my service BACK to my micro SD card and i keep getting stuck at :
"SIM CARD NUMBER - Please enter the SIM card number found in your Straight Talk phone's Prepaid Menu. This number starts with 89."

I've checked and re-checked and it just keeps coming back blank.

I am stuck on the exact page. Is this the same number as is printed on the back of the card? starts with 89 and is 5 digits? -tried carefully 5 times with no luck.

Hi, I'm having this same problem with an iPhone 3GS. I already activated it once, but I had to transfer the service to another phone because the iphone wasn't working, but now I've fixed the iPhone but the activation process on the website is asking for me to enter a SIM Number starting with 89, and I have tried entering the 4 digit number on my SIM card, which does begin with 89, and it isn't doing anything except reload that same page and ask me for my SIM number again.

PLEASE HELP ME!
