Horrible Iphone 4s Internet speeds

I seem to have this problem every month.

I have to go on here and write you guys a message and then You pm me and I send you my sim # and then you guys do something on your side and I reboot my phone and it works.

Well My phone is doing it again. Horrible internet speeds barley works.

I cleared cache and shut all running apps down.

Please help again.
