Unable to use Google Voice Voicemail

I've had a Straight Talk SIM card in multiple different phone for the past 5-6 years. Up until recently (as recent as May 2019) i have been able to forward my voicemail to my Google Voice account. I got a new phone and I had someone call me and tell me that my voicemail is not set up. I am trying to forward to Google Voice using the ##004#GOOGLEVOICENUMBER code and i keep receiving the error that says "Connection problem or invalid MMI code". Is there something wrong with my SIM card? I am on a Google Pixel 3a. Has anyone been able to successfully forward to Google Voice?

I have the same issue. GV voice mail used to work fine. Now my CCF settings seem to have been blanked out, and calls fall through to ST voice mail, which I don't use. Appears to have broken in November. I get errors when I try to use raw MMI commands to reinstate the forwarding and also if I try with the Phone app on my OnePlus 6.

I'm having the same problem with conditional call forwarding - it had been working, and just within the last couple of weeks it's stopped working.
I've tried dialing the *61* code (I'm using the AT&T SIM card, not the T-Mobile, etc SIM card) and I get an error message:
Call forwarding connection problem or invalid MMI code (804)
When I go into my calling settings in Android (calls > 3 dots > settings > calls > call forwarding) I get the error: Call Settings Error Network or SIM card error
Please help!

Hi ST - Mark,
Unfortunately the tech support was not able to help - they tried to tell me that they no long support call forwarding (at all), even though there are still tutorials in their help section, and my daughter-in-law still has conditional call forwarding on her phone/account. They said that they recently stopped supporting it, and when I asked as of when, I was told, and I quote, "since before."
And that was a manager, after I tried explaining it to the first tech that I spoke with and getting no help.
Conditional Call Forwarding must still be an option because it's working on my daughter-in-laws phone/account.
Help! Has anyone had luck with getting Straight Talk to set it up on their account?

I'm having the exact same issue as well (Pixel 4 on Straight Talk Unlimited Plan). I've used Google Voice voicemail for years and it quit working around November 2019. From what I'm reading, this is not a hardware problem and this has happened to plenty other people. Seems that Straight Talk disabled CCF or something. I get this error message every time I try to set my Google Voice voicemail up:
call forwarding
connection problem or invalid MMI code
