Smart Pay Payment Question

I'm a little confused about the payment plan if one purchases a new Straight Talk phone through the Smart Pay program:

According to the website, after a customer is approved into the program, the monthly payment price of the new phone includes the monthly Unlimited plan at $45.
Is that true? Does the monthly payment to Quality One include the required $45 unlimited plan to use the phone? Or, does a customer need to purchase a service plan in addition to the monthly payment for the phone?

Can someone here please clarify? I called customer service, but the rep was unable to help me. First she told me I was speaking about a different program, then she told me that the program I was referring to, clearly visible on the official ST website, didn't exist, then when I asked her to look at the website together, she hung up on me!!

I have used Straight Talk service for almost 10 years. I'm not a new customer who is unfamiliar with how the service works, I just want to know for certain how the Smart Pay program works with my monthly Straight Talk service prior to agreeing to it.
