Samsung Precedent Screen is Frozen

I received my phone in October of 2011..My phone seems to have a mind of its own..for the last two days it has accessed all aspects of my phone by its self..I am not crazy but I have sat there and watched it going through my facebook, my calender..I will be in the process of texting someone and it will just start typing randomly..now the screen will not work at all..I can't even answer my phone..I had transfered my number back to my old straight talk phone but it is taking forever to activate! My fiance is a navy sailor and I have been waiting for a few months to get a call from him and now that he is able to call me my phone doesn't work! I need to know what I have to do to send my phone back for a replacement and if someone would be able to make the porting process faster. This is my only phone so I can't even call tech support.
