rrsdandy , you can follow the instructions below to unblock the PUK:

Press the following keys on your Straight Talk phone: 52785738.
Now press the key below the word OK.
Your phone should display the message "New PIN Code," please enter 1111 and then press OK.
Your phone should display the message " Verify new PIN Code," please re-enter 1111, and press OK.
Your phone screen should display the message "Pin Code Changed." Please remember that your PIN is 1111.
