Issue removing phone from account

How can I remove an active phone from my Straighttalk account?
My mothers phone is on account & she wants to remove it to sign up her own profile to build rewards points.
However, since phone is still active there isn't a tab to remove device.

I am having the same problem. I need to remove my phone from my mother's account and create my own account.
